  • How do I prepare for my first visit?

    • Once you have received confirmation of your appointment and have access to the Patient Portal, please log in and complete the Patient section of the portal, entering as much information as possible. Also, go to our Patient Resources > New Patient Registration page, download and print all the forms found there. Bring the completed forms to your first visit. Be sure to bring a current medication list with you to your first visit. We have a full appointment check list shown on our Appointments page.

  • What do I do if I have an issue after the office is closed or on the weekend?

    • If you are ill or have an urgent medical situation, an on-call physician is available after hours, 24/7. Please click here for After Hours information.

  • What if I need to be admitted to the hospital?

    • All our physicians have admitting privileges to Strong Memorial Hospital.

      • Patients of Drs. Cahn-Hidalgo, Minnella, Chapman, and Yoon are encouraged to use Strong Memorial Hospital.

      • Dr. Tan's patients are encouraged to use Highland Hospital where care will be coordinated through a University of Rochester hospitalist physician. 

  • How do I get my test results?

    • Your test results are easily accessible on MyChart. If you are not registered for MyChart go to mychart.urmc.rochester.edu. Whether or not your are signed up for MyChart, we will contact you regarding your results.

  • How can I get a copy of my medical records?

    • We take your confidentiality very seriously. Your medical records will only be released with your written consent. Consent forms are available in our office or on our Links & Forms page. There is a fee for providing patients with a copy of their records.

  • If I need to be seen on an urgent basis and my doctor is not in the office, who is available to see me?

    • Each day, one doctor is designated as an "on-call" doctor. He or she is responsible for any patient that needs to be seen whose doctor is out of the office. The doctors rotate call responsibilities on a daily basis during the weekdays. In some cases, a patient may be seen by Pamela Damiani, our nurse practitioner who works for Drs. Cahn-Hidalgo and Tan

  • How is my email address being used?

    • If you have provided us with your email address, we are using it to improve your patient experience. You may have used it to sign up for our newsletter, or to pay your bill online. Please contact us if you would like to opt out of our newsletter.
