Pfizer Booster Eligibility

To be given 6 months from their most recent Pfizer vaccine 

All patients 65 years old and older 

If aged 18-64 years old, you must fall into at least one of the following categories: 

Qualifying occupation/living situation

  1. Healthcare workers

  2. First Responders (i.e. firefighters, police, EMS, etc)

  3. Education staff (i.e. teachers, support staff, daycare workers)

  4. Grocery store workers

  5. Public transit workers

  6. Congregant living situation (i.e. group home, correctional facilities)

Qualifying medical condition*

  1. Lung condition (i.e. COPD, asthma, interstitial lung disease, cystic fibrosis, pulmonary hypertension)

  2. Diabetes

  3. Obesity: BMI 26 and above

  4. Heart condition (i.e. heart failure, CAD, cardiomyopathy, hypertension)

  5. Kidney condition (i.e. CKD)

  6. Dementia/Alzheimer’s

  7. Down Syndrome

  8. Liver Disease (i.e. alcohol-related liver disease, nonalcoholic fatty liver disease, cirrhosis)

  9. Pregnant or up to 42 days following end of pregnancy

  10. Sickle cell disease or thalassemia

  11. Smoker: Current or former

  12. Cerebrovascular disease (i.e. history of stroke)

  13. Substance use disorder (i.e. alcohol, opioid, cocaine use)

*The list of qualifying medical conditions does not include rare medical conditions that may still make an individual severely ill if they were contract COVID-19.  Provider discretion can be used in these rare medical conditions. 

**Immunocompromised patients can also continue to get a 3rd Pfizer booster dose as long as it has been 28 days since their 2nd dose.  See separate instructions provided to determine a patient’s immunocompromised status.
